A certificate issued to Krystyna Warecka (Bianka Rozenman, the donor) by the Polish Welfare Committee in Koluszki, stating that the bearer was evacuated from Warsaw to Jedrzejow, but is allowed to join her family in Koluszki.

The certificate was issued on October 16, 1944, during the mass evacuation of the population from Warsaw, following the Polish uprising in August 1944.
